Job summary

Ennoble Care in Little Rock, Arkansas, seeks a Care Coordinator, Embedded at partner dialysis clinics to engage patients in person, coordinate care plans with Ennoble providers and facility staff, schedule visits, and ensure timely follow-up and clear communication with patients, families, and the clinical team.

This full-time on-site role requires clinical certification and strong organization. Proficiency with EMR, Microsoft 365, and teamwork are essential.

Qualifications

Legally authorized to work in the United States of America.
Active, unrestricted clinical certification or licensure such as PCT, CNA, MA/CMA, LPN, RN or comparable credential.
Dialysis-specific certifications are highly preferred (CDT, CHT, CCHT, CCHT-A).
Experience managing provider schedules and coordinating patient appointments.
Minimum 1 year direct experience with hemodialysis/nephrology patients.

Responsibilities

Provide consistent on-site engagement with dialysis patients to support care plans, follow-ups, education, and care transitions.
Coordinate and align patient care using Ennoble Care’s EMR for continuity and timely follow-up.
Schedule patient visits for providers and support daily clinic workflow.
Collaborate with providers, dialysis staff, and a multidisciplinary team for integrated care.
Liaise between providers, patients, families and facility staff with clear communication.
Document patient needs and changes in condition accurately per standards.
