Job summary

Family Health Services in Twin Falls is seeking a Diabetes & Nutrition Education Program Instructor. This full-time role provides individualized DSME/T and MNT if an RDN. Salary ranges from $28.50 to $32.25 per hour DOE, with a quarterly bonus up to $520. You will educate patients, collaborate with the medical team, and use EHR to document progress.

Ideal candidates hold a current RDN, RN, or ACSM-EP license and may have CDEDS credentials. Proficiency in MS Office and Spanish literacy preferred.

Qualifications

• Maintain current unrestricted professional license as an RDN, RN, or ACSM-EP.
• CDEDS credential preferred.
• Ability to counsel with compassion and respect for the individual.
• Must possess initiative and ability to work independently.
• Ability to make knowledgeable clinical assessments and judgments.
• Excellent verbal, written, and presentation skills.
• Proficient in Microsoft Office.
• Spanish literacy preferred.

Responsibilities

• Deliver team-based care in the clinic(s).
• Adapt to interruptions while managing daily tasks.
• Provide comprehensive education including assessments and follow-up plans.
• Evaluate and document progress toward educational objectives.
• Communicate and collaborate with the medical team.
• Collect data for CQI and ADA recognition.
• Provide education and resources to medical staff.
• Utilize EHR and registries to coordinate documentation and ensure patient-centered care.
• Monitor health status and healthcare usage to improve outcomes.
• Follow up with patients as requested by provider.
• Assist in education, support, and care coordination with outside resources.
• Assess readiness to change and address barriers to care.
• Use behavioral strategies to improve self-care and chronic disease management.
• Make reminder calls for patient visits.
• Participate in FHS meetings and committees.
• Perform other duties as assigned.
