Job summary

FMOL Health seeks a Diabetes Coordinator (RN or Dietitian) to oversee the DSME/T program, ensuring delivery of education and adherence to accreditation standards. The role reports to the Wellness Manager and collaborates across departments to optimize patient care.

The candidate should have 5 years of clinical RN or RD experience, at least 2 years in diabetes education, and CDE or equivalent certification, with strong communication and project management skills.

Qualifications

• Experience: 5 years of clinical RN or RD practice with 2 years in diabetes education and certification.
• Education in, and/or experience with, chronic diseases and disease self-management.

Responsibilities

• Develops and oversees the Diabetes Education Program with adherence to standards and documentation for accreditation.
• Builds relationships with community groups, payers and referral sources; liaises with the Diabetes Education Advisory Group.
• Performs patient assessments, educates participants and supports program completion.
• Oversees education delivery to be individualized and fiscally feasible.
• Coordinates marketing activities with Business Development; performs other duties as assigned.
