Job summary

FMOL Clinics is seeking a Diabetes Coordinator (RN or Dietitian) to oversee the DSME/TE program, ensuring coordinated educational services with strong documentation and accreditation. Reports to the Wellness Manager and collaborates with community partners.

The role emphasizes program development, quality improvement, and staff education, with on-site responsibilities at the Gonzales, LA location. Requires RN or RD licensure, CDE, and 5 years clinical experience including diabetes education.

Qualifications

• 5 years clinical experience as RN or RD with 2 years in diabetes education
• Certification in Diabetes Education
• Education in and/or experience with chronic diseases and disease self-management
• Bachelor's degree from an accredited school
• Knowledge of chronic disease management and education processes
• Licensure as RN or RD with CDE certification and BLS certification

Responsibilities

• Oversees planning, implementation and evaluation of the DSME/T program
• Maintains documentation in electronic medical records and national program accreditation
• Develops relationships with community groups, payers and referral sources
• Ensures quality program evaluations and ongoing education for staff
• Coordinates marketing activities with Business Development
• Performs other duties as assigned
