Job summary

FMOL Clinics is seeking a Diabetes Coordinator (RN or Dietitian) to oversee the DSME/T program, ensuring coordinated diabetes education services and ongoing accreditation readiness. Reports to the Wellness Manager and collaborates with the community and internal teams.

The role emphasizes program development, quality oversight, and cross-functional collaboration, with a focus on patient-centered education and effective documentation.

Qualifications

• RN or RD with 5+ years clinical experience; 2+ years in diabetes education.
• Certification in Diabetes Education required.
• Education in chronic disease management and self-management.
• Strong verbal and written communication, interpersonal, critical thinking, organizational, time management and computer skills.

Responsibilities

• Diabetes Education Program Development/Oversight: plans, coordinates, implements and evaluates activities and curriculum; maintains documentation in EMR and statistics for accreditation.
• Quality: conducts program evaluation, performs age-appropriate assessments, ensures quality standards and ongoing professional development.
• Collaboration & Partnership: builds relationships with community groups, payers, referrals and advisory groups.
• Other Duties: coordinates marketing with Business Development and performs additional tasks as assigned.
