Job summary

Boston Medical Center in Boston, MA is seeking an Outreach Specialist for the HEART of Communities program. You will be the first point of contact for prospective participants, conducting outreach and enrollment activities while coordinating with Community Wellness Advocates and clinical teams.

You will help develop outreach workflows, support enrollment goals, and manage program data in Epic. This role also supports special projects under the Operations Manager and requires strong communication

Qualifications

• Strong oral and written communication skills.
• Proficiency with software applications including Microsoft Office and Epic.
• High school diploma with at least 1 year of customer service experience or equivalent

Responsibilities

• Conduct outreach calls and chart review for new eligible HEART patients.
• Schedule intake appointments and assist with rescheduling for no-shows or cancellations.
• Document baseline data in Epic and maintain program database for new patients.
• Assist with outreach data management, develop efficient workflows, and document SOPs.
• Support HEART special projects as assigned by the Operations Manager.
