Qualifications

• Experience in diabetes education and DSMES program delivery.
• CDCES certification preferred.

Responsibilities

• Teach diabetes self-management, glucose monitoring, medications, nutrition, activity and risk reduction; document education.
• Collaborate with leadership, intake, credentialing, billing and quality teams.
• Maintain confidentiality, respect participant choice and follow professional scope.
