Job summary
Grace Community Care and Homes Inc. is seeking a part-time Diabetes and Nutrition Program Lead in New Jersey to advance person-centered diabetes education, nutrition planning and prevention workflows.
The role involves developing workflows, coordinating with credentialing, billing and quality teams, and monitoring participant outcomes in a collaborative environment. Applicants should hold a relevant licensed clinical or registered nutrition credential, preferably with CDCES or comparable
Qualifications
- Licensed clinical nutrition credential required.
- Diabetes program leadership experience preferred.
- CDCES or equivalent expertise preferred.
Responsibilities
- Develop diabetes education, nutrition and prevention workflows, coordinate recognition requirements, staff training and referral partnerships, and monitor participant outcomes.
- Work collaboratively with department leadership, intake, credentialing, billing and quality teams.
- Maintain confidentiality, respect participant choice, communicate service gaps promptly, and follow role-specific policies and professional scope.
- Participate in orientation, competency review and quality improvement.
