Job summary

Grace Community Care & Homes is developing its Diabetes and Nutrition department and seeks a Diabetes and Nutrition Program Lead. The role will contribute to person-centered services, reliable operations and accurate documentation.

This is a part-time role in New Jersey with on-site or community assignments as applicable. Start date depends on program approvals and staffing readiness. Clinical services begin only at approved locations and within scope.

Qualifications

• Diabetes program leadership experience is required.
• CDCES or comparable expertise preferred.
• Strong collaboration with interdisciplinary teams and credentialing/billing knowledge.

Responsibilities

• Develop diabetes education, nutrition and prevention workflows.
• Coordinate staff training and referral partnerships.
• Monitor participant outcomes and ensure accurate documentation.
• Collaborate with department leadership, intake, credentialing, billing and quality teams.
