Organization: Group Health Cooperative of South Central Wisconsin - BETTER TOGETHER

Positions: 1.0 FTE / 40 hours per week, with consideration for 0.8 FTE / 32 hours per week. The role will split time between Sauk Trails Clinic and East Clinic.

Role Summary

The Diabetes Educator provides diabetes-related care and education to members with diabetes, coordinating with practitioners, nursing, and health education staff. Responsibilities include clinical care, patient education on diabetes management, diet and exercise modification, blood sugar monitoring, coping strategies, problem-solving, and risk reduction. The role also involves medication management under physician protocols.

The incumbent adheres to GHC-SCW policies, maintains high-quality standards, performs duties timely, and promotes the organization's Mission, Vision, and Values.

Minimum Qualifications
  • Graduate of an accredited nursing or dietician program.
  • State licensure as a Registered Nurse or Registered Dietitian.
  • Certification as a Diabetes Educator (CDCES) or working towards it, to be obtained within one year.
  • Preferred: Two years managing diabetic patient care.
  • Valid Healthcare Provider CPR/AED certification, to be obtained within four months.
  • Ability to travel between work sites; valid Wisconsin driver’s license required.
  • Knowledge of counseling, cultural sensitivity, patient assessment, care coordination, confidentiality, HIPAA, patient education material development, and computer skills including EMR systems like EPIC.
  • Excellent communication skills, professionalism, and flexibility to work early evenings as needed.
