Organization: Group Health Cooperative of South Central Wisconsin - BETTER TOGETHER
Positions: 1.0 FTE / 40 hours per week, with consideration for 0.8 FTE / 32 hours per week. The role will split time between Sauk Trails Clinic and East Clinic.
The Diabetes Educator provides diabetes-related care and education to members with diabetes, coordinating with practitioners, nursing, and health education staff. Responsibilities include clinical care, patient education on diabetes management, diet and exercise modification, blood sugar monitoring, coping strategies, problem-solving, and risk reduction. The role also involves medication management under physician protocols.
