Job summary

MACT Health Board, Inc. is seeking a qualified Diabetes Program Registered Nurse for a full-time clinical role.

The Diabetic RN will travel to MACT clinics, provide DSMES services and ADA curriculum, and coordinate diabetes care across locations to improve patient outcomes. You will develop and monitor program targets, ensure compliance with ADA, GPRA and grant requirements, manage records in the EHR, and educate patients through diabetes care classes.

Qualifications

R.N. licensure with Diabetes Educator Licensure or license eligibility within two years.
Knowledge of community diabetes resources; care for patients of all ages.

Responsibilities

Travel to MACT clinics to assess diabetic patients and coordinate care across sites.
Provide DSMES services and ADA curriculum across locations.
Plan and develop the Diabetes Program including outcome targets and measures.
Monitor GPRA/GPMS compliance and grant requirements.
Prepare grant reports and assist with budgets and funding opportunities.
Develop and maintain diabetes care activities and individualized patient care plans.
Represent the Diabetes Program at meetings and community health events.
Update the diabetic work plan annually.
Educate patients with diabetes and conduct care classes.
Maintain medical records in the EHR and ensure timely documentation.
