Job summary

The Diabetes Care and Education Specialist at Citizen Potawatomi Nation Health Services delivers comprehensive diabetes self-management education and medical nutrition therapy, bridging clinical and self-management for optimal outcomes. The role works with a multidisciplinary team to tailor plans, assess needs, and promote patient-centered care.

Responsibilities include documentation in the EHR, collaboration with primary care providers, population health management, and contributing to quality

Qualifications

• Bachelor’s degree in nursing, dietetics/nutrition/pharmacy
• Licensed in the State of Oklahoma
• CDCES or willingness to obtain certification within 24 months of employment
• Experience working with patients with diabetes is preferred, if applicable

Responsibilities

• Provides quality diabetes self-management education and medical nutrition therapy (limited to Registered Dietitians) in individual and group settings.
• Completes comprehensive assessments for each patient, interprets data, and develops individualized care plans.
• Documents all contacts/visits in the EHR and outcomes database per policy.
• Collaborates with other diabetes care team members to develop person-centered plans.
• Advocates for technology-enabled diabetes education and care.
• Work closely with Primary Care Providers to manage medications, labs, and triage as needed.
• Coordinate population management of the diabetes patient panel.
• Assist patients in obtaining medications and supplies.
• Participate in quality improvement, quality assurance, and innovation activities.
