Qualifications

• Bachelor's degree required.
• At least 2 years of relevant clinical diabetes education experience.
• Must have NY State RN, CDN, or RDN licensure.
• Certified Diabetes Care and Education Specialist.
• Current BCLS certification.
• Current Infection Control Certification.
• Master's degree preferred.
• Bi-lingual Spanish preferred.

Responsibilities

• Provide diabetes self-management education and support.
• Coordinate care with multidisciplinary team.
• Assess patient risk factors and develop care plans.
• Monitor and document patient status and progress.
• Educate on healthy eating, physical activity, and glucose monitoring.
• Refer to community resources and assist patients in utilizing them.
