Job summary

Cabarrus Rowan Community Health Centers is seeking a dedicated Certified Diabetes Educator to empower patients with pre-diabetes and diabetes through self-management education, lifestyle modification, and clinical guidance. This role also requires traveling to APHS System clinics; reliable transportation is required.

Licensure as an RN, RDN, Pharmacist, or other allied health professional is required, with CDCES credential preferred or eligible soon.

Qualifications

• Current, active license as an RN, RDN, Pharmacist, Exercise Physiologist or other allied health professional.
• CDCES credential preferred or eligible to sit for the exam within a specified timeframe.
• 1–2 years clinical diabetes education experience preferred.

Responsibilities

• Maintain accurate documentation in the patient’s EHR.
• Obtain lab/X-ray reports and referral information; verify demographics.
• Maintain patient confidentiality and protect PHI.
• Assist medical providers with patient communication about medications and symptoms.
• Ensure culturally and linguistically appropriate services.
• Participate in in-services, trainings, and quality improvement activities.
• Maintain a safe, compliant work environment and infection control.
