Job summary

Trinity Health's St. Peter's Hospital in Albany, NY, is seeking a Part-time Community Health Worker for the Day shift to support diabetes prevention and non-clinical needs.

The CHW will liaise between patients and providers, conduct assessments, assist with referrals, and promote health literacy. A high school diploma is required, with associates degree preferred, and at least one year of community involvement.

Qualifications

• High School Diploma required; Associates degree preferred.
• Minimum one year of community involvement experience.
• Ability to earn a CHW certificate is preferred.
• Bi-lingual skills preferred; medical interpreter/translator is a plus.
• Ability to work in a collaborative, shared leadership environment.

Responsibilities

• Conducts and interprets individual assessments to identify non-clinical needs and mitigate barriers to resources and services.
• Improves access to health and human services through in-person interactions, telephone contacts, and coordination of referrals per protocols.
• Reinforces health literacy and educational messages using appropriate tools and supplies within scope of practice.
• Serves as a liaison between providers and patients to facilitate communication and coordination of services.
• Continuously expands knowledge of community services and resources; maintains frequent contact with community agencies and participates in community meetings.
• Uses Social Care tools via TogetherCare (or similar protocols) for care coordination.
• Assists with data collection.
• Screen and mitigate social care needs of dually eligible patients (Medicaid/Medicare) as an intervention to reduce preventable hospitalizations.
• Address and mitigate social care needs for colleagues as part of the Colleague Resiliency Plan.
