Job summary

Brandywine Counseling & Community Services, Inc. is seeking a DFS Liaison/Case Manager to support families and connect clients to appropriate services in the community. This role emphasizes transport, outreach, screening, and collaboration with DFS staff.

You will work primarily in the field, coordinating admissions and benefits while maintaining strong communication with clients and teams.

Qualifications

• Knowledge of screening tools and therapy principles.
• Considerable knowledge of the population served and available resources.
• Ability to screen clients for appropriate services and document progress.
• Crisis intervention may be required in emergency situations.
• Skill in establishing rapport with clients.
• Ability to work independently and as part of a focused team.
• Ability to communicate effectively orally and in writing.

Responsibilities

• Support families to prevent the use and misuse of drugs and the development of substance use disorders.
• Promote child safety by identifying substance use and connecting families to interventions or treatment programs.
• Reduce the risk of future harm to children by addressing the substance use needs in families.
• Transport clients to appointments in the community and BCCS offices.
• Provide liaison, support and educational services to DFS staff.
• Participate in case presentations at weekly meetings.
• Contribute to client screening processes prior to case management.
• Provide outreach activities to the community.
• Administer various screening tools and explain program rules.
• Assist clients in obtaining admission, entitlements, and related benefits.
• This is primarily a community-based position; work will be outside of an office setting up to 100% of the time.
