Job summary

Far-Northern-Regional-Center in Chico, CA is seeking a Service Coordinator to provide case management services to individuals with developmental disabilities. You will develop and monitor Individual Program Plans and coordinate services with other agencies and providers.

The position requires a B.A. degree in Social Welfare or equivalent experience, and includes home visits, client advocacy, reporting, and maintaining case records. Travel for field work is included.

Qualifications

• Requires equivalent to a B.A. Degree in field related to Social Welfare or one year of experience in a like position.

Responsibilities

• Provide case management services to an assigned group of persons with developmental disabilities.
• Develop and monitor Individual Program Plans.
• Coordinate services with other agencies and service providers.
• Conduct home visits with clients, families or other care providers.
• Consult with clinical and other professional staff.
• Advocate on behalf of clients.
• Complete reports and maintain case records.
