Job summary

The State of Minnesota is seeking a Designated Coordinator (Skills Development Specialist) in Willmar, MN. This part-time, nonexempt role supports individuals with cognitive/intellectual disabilities and co-occurring conditions under 245D guidelines, coordinating individualized plans and staff training.

Qualifications

• 4 years of full-time work experience providing direct care services to persons with disabilities or persons 65 years and older.
• Minimum of 50 hours of education and training related to human services and disabilities.
• Diploma in a field related to human services from an accredited post-secondary institution and 3 years full-time work experience providing direct care services to persons with disabilities or persons 65 years and older.
• Associate degree in a field related to human services and 2 years full-time work experience providing direct care services to persons with disabilities or persons 65 years and older.
• Bachelor’s degree in a field related to human services and 1 year full-time work experience providing direct care services to persons with disabilities or persons 65 years and older.
• Human Services related degrees may include: Family Social Sciences, Health Care Management, Special Education, Public Health, Recreational Therapy, Social Work, Alcohol and Drug Counseling, Social Science, Health Science, Psychology.
• Must have a valid Driver’s License
• Must be at least 18 years of age

Responsibilities

• Using a person-centered approach; development, implementation, review and revision of individualized program plans that support individuals in achieving their goals and dreams.
• Providing guidance, training and feedback to staff to ensure the highest standards of service delivery are provided to the individuals.
• Providing direct care to individuals such as job coaching, community integration, personal cares (as needed), medication/treatment administration, and behavior interventions.
• Contributing to the safety and security of the site, individuals served and co-workers.
