Qualifications

• Associate's or Vocational degree graduate of an accredited school of nursing.
• Internal clinic staff who have functioned successfully as a CMA or LPN with 3+ years may be considered from an accredited school that does not have clinical rotations where department allows 0 to 2 years RN experience.
• Bachelor's degree in nursing is preferred for some roles; RN licensure required.

Responsibilities

• Assess data to determine nursing diagnoses and plan patient care.
• Identify outcomes and establish realistic, measurable goals with patients and families.
• Develop and implement an individualized care plan with evidence-based practices.
• Coordinate care with the health care team and community resources.
• Document nursing diagnoses, interventions and patient responses per guidelines.
• Evaluate progress and revise diagnoses and plans as needed.
• Provide patient advocacy and participate in quality improvement activities.
