Job summary

Human Touch Home Health Care is seeking an Occupational Therapist to implement standards of care for home health OT services, with duties including evaluating functional status, home environments, and developing care plans. This PRN role requires state-appropriate OT licensure, NBCOT registration, CPR, and a valid driver’s license.

The therapist will collaborate with physicians and staff, educate patients and families, and maintain accurate documentation while delivering care in patients’ homes

Qualifications

• OT degree from an accredited curriculum.
• Registered with NBCOT.
• Two years of clinical OT experience.
• Current CPR certification.
• Licensed driver with reliable vehicle.

Responsibilities

• Evaluates patient’s functional status and home environment to plan care.
• Develops and communicates plan of care and goals.
• Performs work capacity evaluations for return-to-work cases when applicable.
• Fabricates splints and trains patients in adaptive equipment use.
• Trains patients in prosthetic/orthotic device use and safe daily activities.
• Maintains clinical records and communicates changes to physicians.
