Job summary

LiveWell Homecare Agency, LLC is seeking a Home Health Occupational Therapist to assess patients' daily living and work-related skills, and to develop and implement therapeutic retraining programs.

You will collaborate with physicians and healthcare teams to create and revise care plans, provide treatments to improve strength and range of motion, and design community reintegration activities, including adaptive equipment recommendations to promote independence.

Supervising and instructing occupational therapy assistants and aides, documenting patient progress, and participating in discharge planning are key responsibilities.

Qualifications include:

Preferred experience includes at least six months in occupational therapy and home health settings.

This role involves patient-facing activities, adherence to TB screening, and driver safety programs.

Compensation ranges from $85,400 to $117,500 annually, with benefits supporting overall well-being.
