Job summary

Area Agency on Aging and Community Action Agency for Northwest Indiana, CoAction is seeking a Dementia Outreach Specialist to implement the DCSP across the region. This role focuses on reducing health disparities, improving dementia-related outcomes, and educating staff and partners about dementia care resources.

The position emphasizes collaboration with hospitals, providers, and community organizations to expand dementia services and referrals, while ensuring confidentiality and HIPAA

Qualifications

• Expertise and knowledge regarding dementia, dementia care and support, family and informal caregiver needs and challenging behaviors.
• Experience in dementia screening, care, caregiver education, and community awareness programming preferred.
• Experience working with older adults in healthcare or community settings preferred.

Responsibilities

• Maintain participant and family confidentiality and HIPAA compliance.
• Work independently, prioritize tasks and handle multiple duties.
• Educate CoAction staff and the public on dementia per DCSP guidelines.
• Develop referral protocols with healthcare providers and community partners.
• Provide public outreach and training on dementia to staff and general public.
• Complete timely program data reporting and DCSP evaluation.
