City of Baltimore seeks a Dementia Navigation Program Assistant to provide administrative, programmatic, and outreach support for dementia-related services. The role connects callers to resources, coordinates educational programs, and assists with Maryland Access Point call center duties when needed.

The position requires a bachelor's degree and 1 year of relevant experience; preferred: associate degree and 2+ years in admin, outreach, or health-related fields.

Qualifications

• Bachelor's degree required; one year coordinating health/social services cases preferred.
• Experience coordinating, monitoring, and processing cases involving health, social, vocational or rehabilitation services.
• Excellent communication and documentation skills; ability to handle confidential information.

Responsibilities

• Provide administrative support for the dementia navigation program.
• Answer calls and emails from individuals, caregivers, and partners promptly and professionally.
• Connect families with community resources and referrals based on needs.
• Coordinate event registrations and follow-up communications.
• Support outreach to community organizations, healthcare providers, and partner agencies.
