Job summary

Oakwood Creative Care in Mesa, AZ, is seeking a compassionate social worker to join our team. You will partner with families to design personalized plans that honor routines and relationships, provide dementia education and caregiver coaching, and help individuals remain safely at home and in their communities.

Ideal candidates hold a BS in Social Work (MSW preferred), have 2+ years in geriatric or dementia care, and are comfortable using electronic health records.

Qualifications

• Bachelor's degree in Social Work (BSW) required.
• Master's degree in Social Work (MSW) strongly preferred.
• 2+ years of experience working with older adults, people living with dementia, and/or family caregivers.
• Familiarity with community resources for older adults in the Maricopa County area.

Responsibilities

• Get to know the families you serve and build personalized plans.
• Provide dementia education, caregiver coaching, and practical strategies.
• Help patients stay in the home and community as long as possible.
• Connect patients and families to community resources and services.
• Stay connected with patients and caregivers by phone, video, or in person.
• Respond calmly during crises or transitions.
• Collaborate with interdisciplinary team and community partners.
• Facilitate education classes and support groups.
• Document work in the electronic health record system.
