Qualifications

• 2–5 years of experience working with older adults with memory and cognitive impairments.
• Licensure (LSW or LMSW) is required for this role.
• Experience with medically at-risk populations is preferred.

Responsibilities

• Complete in-home intake assessment for program participants.
• Develop and update care plans with input from the program team.
• Assist participants and families in crisis with needed support and resources.
