Baylor College of Medicine in Texas Medical Center, Houston, seeks a compassionate Social Worker for the Alzheimer's Disease and Memory Disorders Clinic. You will perform psychosocial assessments, provide counseling, coordinate care, and navigate community resources for patients and families.

Ideal candidates have about a year in social work, some Alzheimer’s exposure, and strong collaboration skills. Epic EMR experience is preferred; this role is on-site at the McNair Campus.

Qualifications

• Bachelor's degree in Social Work and licensure as LBSW or LMSW in Texas.
• At least one year of relevant social work experience, preferably in dementia care.
• Familiarity with community resources, crisis intervention, and care coordination.

Responsabilités

• Conduct psychosocial assessments for patients and caregivers and develop care plans.
• Provide counseling and crisis intervention; connect families with resources and services.
• Coordinate with interdisciplinary teams and document in the EMR.
• Support advance care planning and caregiver education throughout disease progression.
