Delaware Hospice is seeking a full-time Dementia Care Navigator in Milford, DE. You will serve as the primary non-clinical contact for people living with dementia and their caregivers, providing relationship-based support and navigation using standardized workflows and technology-enabled tools.

You will collaborate with the interdisciplinary team to ensure high-quality dementia care, caregiver engagement, and scalable program delivery, in a hybrid in-person/remote capacity.

Qualifications

• Minimum two years of experience in care coordination, patient navigation, case management, or healthcare-related service delivery.
• Experience with older adults, dementia, and/or family caregivers preferred.
• Dementia Care certification/training required (provided through Delaware Hospice).

# DEMENTIA CARE NAVIGATORMilford, DE 19963## Description**Delaware Hospice is actively seeking a full-time Dementia Care Navigator.**The **Dementia Care Navigator** is a key member of the Delaware Hospice Dementia Care GUIDE Program, serving as the primary non-clinical point of contact for individuals living with dementia and their caregivers. The role provides relationship-based caregiver support, care coordination, and navigation services using standardized workflows and technology-enabled tools, in alignment with the CMS Guiding an Improved Dementia Experience (GUIDE) Model. The Care Navigator works collaboratively with the interdisciplinary team members to support high-quality, consistent dementia care delivery, caregiver engagement, and program scalability.
