Job summary

Delaware Hospice is seeking a full-time Dementia Care RN to coordinate clinical care for community-dwelling individuals with dementia. You will perform remote assessments, educate family caregivers, and work within an interdisciplinary team to prevent hospitalizations.

The role emphasizes telehealth and in-person care, with flexible scheduling to support patients in Sussex County and surrounding areas. New grads welcome; hospice heart and training provided.

Qualifications

• RN licensure required; geriatrics, neurology, or dementia care experience preferred.
• Comfort with telehealth and remote assessments; patient-centered care.

Responsibilities

• Provide clinical care coordination and remote assessments for Medicare-enrolled individuals with dementia.
• Educate patients and family caregivers; collaborate with care navigators, physicians, and other team members.
• Deliver continuous, personalized support in person and virtually to reduce hospitalizations or nursing home placements.
