Job summary

Avamere at Chestnut Lane in Gresham, OR seeks a part-time on-call Support Services Provider for the Deaf and DeafBlind community. The role bridges communication gaps, develops support plans, facilitates access to services, and advocates for residents' accessibility rights within the facility and the community.

Responsibilities include coordinating sign language interpreters, assisting with daily correspondence, planning ISPs, helping with orientation and mobility, and promoting independence

Qualifications

• Bachelor’s degree or equivalent knowledge of state regulations for assisted living preferred.
• Exceptional fluency in American Sign Language and familiarity with tactile sign language, Pro-Tactile.
• Minimum 1–3 years of relevant experience with elderly or disabled individuals, preferably in deaf/deaf-blind communities.
• State certification (SSP) and active CPR/BLS during employment.
• Excellent verbal and written communication with residents, families, and staff.

Responsibilities

• Communication Access & Facilitation: relay information using preferred communication mode (ASL, tactile, braille) and coordinate interpreters and captioning.
• Care Coordination: develop ISPs, connect residents with outside services, review health documentation with nurses.
• Independent Living: provide orientation and mobility coaching, plan recreational activities and manage outings.
• Advocacy & Training: educate staff on deaf culture, advocate for accessibility, ensure compliant care.
