Job summary

St. John’s Community Services is hiring a Program Coordinator to oversee multiple group homes and residential settings. You will supervise staff, coordinate ISPs, and liaise with providers to ensure quality services for participants.

The role includes managing finances, conducting visits, and training staff while ensuring compliance with state and federal requirements. 50-65% travel may be required.

Qualifications

  • Bachelor’s degree as regulations requires per state.
  • Experience working with people with disabilities.
  • Valid health certificate that meets jurisdiction requirements.
  • Valid local driver’s license with good driving record.
  • Reliable means of transportation.
  • Ability to pass a criminal background check.

Responsibilities

  • Supervise day-to-day operation of multiple group homes or residential settings.
  • Coordinate and write Individual Service Plans (ISPs) and ensure implementation.
  • Liaise with community providers, case managers, and inter-departmental services.
  • Oversee finances for the consumers in the homes and conduct visits.
  • Train staff and ensure program practices are followed; provide 24/7 on-call coverage.

As the Program Coordinator you will be expected to monitor and evaluate all aspects of assigned program operations to ensure quality of service for all involved participants; ensures that facilities are maintained safely and cleanly and in accordance with the applicable regulations. Supervises and coordinates the day-to-day operation of the group home/residential setting and performs other job related duties as assignedYour overall essential responsibilities will be the following:Supervise the day-to-day operation of multiple group homes and/or residential settings assigned.Supervises the Residential Team Leaders.Serve as liaison with community providers, case managers, group homes, parents and inter-departmental services to facilitate the coordination of services to the participants as requested, as well as increase and improve participant involvement.Ensure that the homes are meeting the state, federal, and local requirements.Acts as the Lead Clinical Coordinator of the ISP team.Coordinate and write the Individual Service Plans and ensure implementationOversee the financial needs of the consumers in the homes.Conducts monthly and regular visits to the day program.Ensure that all medical appointments and treatments are maintained.Prepare and review relevant documents and reports.Coordinate in-house staff training events and conduct training on the ISP implementation and documentation.Train and evaluate staff.Ensure staff compliance with all acceptable program practices and procedures.Ensures quality care in regard to the personal needs; clothing, hygiene, food, supplies, and home environment for the individuals served.Troubleshoot and/or resolve any issues or areas of concern that arise when dealing with the community.Attend and participate in all relevant agency events, workshops, training and meetings.Provide coverage when there is a staffing need.Provide 24/7 on-call coverage for assigned locations.This list of responsibilities, skills, efforts, or working conditions associated with this position is not in its entirety as other duties may be assigned and are expected to be followed.All SJCS employees must:Obtain a valid health certificate to meet SJCS or current funding authority requirements as required.Meet SJCS and the state funding authority requirements of criminal background and record checks.Physical and environmental conditions include:May lift a minimum of 35 pounds, to position and/or transfer persons with disabilities.Able to sit, stand, bend, stoop, reach and walk for long periodsMay be required to travel up to 50-65%. With possible overnightsWork environment involves risks or discomforts, which require special safety precautions e.g. safely operating a motor vehicle, exposure to aggressive human behavior.
