Job summary

Position Description

The DDS LPN is primarily responsible for providing various supports to individuals with disabilities in a group home setting. The LPN assists individuals in all areas of daily living including personal care, housekeeping, cooking, and recreational activities. The LPN provides medical treatment beyond the scope of a MAP certified direct care staff. Work closely with individuals to help them become as independent as possible in order to meet their own personal goals. Assist individuals in finding and participating in community activities they enjoy while encouraging community membership and individual interests.

Qualifications

• License by the MA Board of Nursing as a Licensed Practical Nurse and have 2 years’ experience in the direct care of the elderly or disabled.
• Ability to complete all initial and ongoing background checks including but not limited to: CORI, SORI, Fingerprinting, LEIE, Driving record, E‑Verify, Physical, PPD and licensing and/or certification checks (i.e., nursing).
• Valid US driver’s license and ready access to a vehicle.
