Job summary

The Montana Department of Public Health and Human Services (DPHHS) is seeking a DDP Targeted Case Manager in Billings. The role focuses on comprehensive assessments, development and revision of care plans, and referrals to ensure members obtain needed services and quality is maintained.

Qualifications include a BA in social work or related field and at least two years in human services, with DD experience preferred. The position may offer partial telework within Montana.

Qualifications

Bachelor’s degree in social work or a related field from an accredited college.
Minimum of two years of experience in human services, with at least one year in the field of developmental disabilities (DD).
Ability to follow written and verbal directions; communicate effectively both orally and in writing.
Ability to facilitate, negotiate and mediate problems and promote team consensus.

Responsibilities

Perform comprehensive assessment and periodic reassessment of individual needs.
Develop and revise the plan of care; coordinate referrals and related activities.
Monitor and follow up to ensure the quality of the plan of care.
Ensure the plan of care aligns with program requirements and outcomes.
