Job summary

The Montana Department of Public Health and Human Services (DPHHS) seeks a DDP Targeted Case Manager in Helena to perform comprehensive assessments, plan development, and service coordination for individuals with developmental disabilities.

Partial telework within Montana may be available; travel up to 25% is expected.

Qualifications

• Bachelor’s degree in social work or related field from an accredited college.
• Minimum of two years of experience in human services, with at least one year of experience in the field of developmental disabilities.
• If lacking one year of experience in the field of DD, complete at least 40 hours of training in service delivery to persons with DD within three months of hire or designation as a case manager.

The Montana Department of Public Health and Human Services (DPHHS) has a career opportunity within the Behavioral Health & Developmental Disabilities Division.

The Developmental Disabilities Program procures and supports community-based services for people with developmental disabilities and/or their families.

The DDP Targeted Case Manager is responsible for a comprehensive assessment and periodic reassessment of individual needs, the development of and periodic revision of the plan of care, referral and related activities to help the member obtain needed services, and monitoring and follow-up activities. The Case Manager is responsible to ensure the quality of the plan of care.

Additional Information

This position may be eligible for partial teleworkwithin the state of Montana.Specific conditions will be provided by the supervisor and must adhere to state policy.
