Job summary

Klamath County Developmental Disabilities Services is seeking a Services Coordinator to provide targeted case management for individuals with I/DD in Oregon. You will assess needs, develop plans, and coordinate access to medical, employment, social, and educational services.

This role requires knowledge of Oregon DD services, a valid Oregon driver’s license, and commitment to annual I/DD training. You will work under the Developmental Disabilities Services department, maintain confidentiality,

Qualifications

• Must have knowledge of the public service system for developmental disabilities services in Oregon.
• Core competency training sequence approved by ODDS and KCDDS.

Responsibilities

• Assess and reassess service needs and develop ISP/Annual Plan based on assessment.
• Access services through referrals and monitor implementation of ISP/Annual Plan.
• Deliver person-centered services and coordinate with providers and community partners.
• Maintain knowledge of laws, rules, policies, and regulations relevant to DD services.
• Maintain a caseload and participate in transition planning and IEP-related activities.
• Protect confidentiality and fulfill mandatory abuse reporting responsibilities.
