Job summary
Kern Regional Center is seeking a dedicated Service Coordinator/Case Manager to provide comprehensive service coordination and advocacy for individuals with developmental disabilities. You will develop and implement IPP/PCP plans, complete progress reports, and conduct annual reviews for clients in traditional or Self-Determination Programs.
The role involves follow-ups, reassessments, community resource referrals, and purchase/monitoring of vendored services while ensuring adherence to state
Qualifications
- BA/BS degree in human services or related field and two years of relevant experience.
- Bilingual Spanish/English considered highly desirable.
- Detail-oriented and highly organized with strong communication skills.
- Proficient with standard office software and DMV requirements.
Responsibilities
- Provide overall service coordination and advocacy for consumers and families.
- Develop and implement IPP and PCP; prepare progress reports and annual reviews.
- Conduct follow-ups, reassessments and periodic progress reviews.
- Identify community resources, provide information and referrals.
- Initiate vendored services and monitor service delivery; stay compliant with regulations.
