Job summary

Prince William County Community Services is seeking a full-time Service Coordinator (Clinical Services Caseworker) to manage a caseload of individuals with developmental disabilities. You will conduct intakes, needs assessments, and service plans, coordinating treatment teams and monitoring care quality to ensure outcomes.

The role requires a bachelor’s degree in a human services field and at least two years of relevant experience, with opportunities for supervision training and loan forgiveness

Qualifications

• Bachelor’s degree in a human services-related field and 2 years of experience providing service coordination/case management for individuals with developmental disabilities.
• Experience with Medicaid State Plan Option and Developmental Disability Waivers is preferred.
• Knowledge of person-centered practices, DD/MH, and collaboration with community partners is desirable.

Responsibilities

• Maintain a caseload of individuals needing services from community providers or CS programs.
• Provide case management including intakes, needs assessments, and service plan development.
• Link, monitor, refer, coordinate, and facilitate services needed by individuals.
• Conduct and use assessments for individual service plan development.
• Coordinate and facilitate treatment team meetings and revise service plans as needed.
• Monitor quality of care through home/community visits and ensure documentation of services.
