Weakley Nursing & Rehabilitation Center is a skilled nursing and rehabilitation facility committed to providing high-quality, person-centered care to our residents. We foster a team-oriented environment where employees are supported, appreciated, and encouraged to grow professionally.
Provide direct nursing care, including medication administration, treatments, assessments, wound care, and documentation.
Monitor resident conditions and promptly report changes to the RN and physician.
Supervise and support Certified Nursing Assistants (CNAs) in delivering quality resident care.
Communicate effectively with residents, families, physicians, and interdisciplinary team members.
Maintain compliance with facility policies, infection control practices, and state and federal regulations.
12-Hour Day Shift
6:00 AM – 6:15 PM
