Corporation for Public Management is seeking a Day Support Specialist to help individuals build skills, relationships, and independence. You will create engaging activities, supervise participation, and support personal growth in community settings.

The ideal candidate communicates well, takes initiative, and provides respectful, person-centered care. A valid driver’s license, reliability, and experience with individuals with disabilities are preferred.

Qualifikationen

• High school diploma or GED required.
• Valid driver’s license and reliable transportation.
• Knowledge of Connecticut DDS systems preferred.
• DDS Medication Certification, First Aid, CPR, and PMT certifications within six months of hire.

Aufgaben

• Actively engage individuals in meaningful activities within program and community settings.
• Teach, assist, and supervise individuals during activities, employment opportunities, and skill-building exercises.
• Plan community outings and provide individuals with multiple activity options.
• Assist with personal care while maintaining dignity, privacy, and safety.
• Support individuals in achieving goals identified in their ISP.
• Document ISP objectives, attendance, behavioral data, participation, and progress.
• Maintain organized and current case records.
• Report behavioral, medical, or routine changes to the supervisor or team.
• Safely transport individuals between locations and keep vehicle logs.
