Job summary

Civic Center Health and Rehabilitation is seeking a skilled Registered Nurse (RN) to provide direct care to residents in a long-term care and rehabilitation setting. The RN will support rehabilitation plans and collaborate with the clinical team to uphold high standards of care.

Qualifications

Valid Registered Nurse (RN) license.
Strong geriatric and rehabilitation care knowledge.
Excellent communication and interpersonal skills.
Ability to work collaboratively in a team-oriented environment.
Current BLS/CPR certification required.

Responsibilities

Deliver patient-centered nursing care to residents in a long-term care and rehabilitation setting.
Assess, monitor, and document residents' health conditions and progress.
Administer medications and treatments as prescribed, ensuring compliance with facility protocols.
Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans.
Provide education and support to residents and their families regarding care plans and health conditions.
Maintain accurate and timely documentation in compliance with state and federal regulations.
Foster a safe, comfortable, and therapeutic environment for residents.
Supervise and mentor nursing assistants to ensure quality care delivery.
