Job summary

Mercyhealth is seeking a Physical Therapist to evaluate, plan, and implement treatment addressing functional needs across varied diagnoses. Schedule is 24 hours per week with days from 7am to 6pm.

You will collaborate with the care team, document electronically, and participate in performance improvement initiatives. Requirements include graduation from an accredited PT program, and licensure in WI, IL or both if required, with CPR/BLS within 90 days of hire.

Qualifications

• Licensed Physical Therapist in WI, IL or both if required.
• CPR/BLS certification within 90 days of hire.
• Independent judgement within professional standards.

Responsibilities

• Integrates subjective and objective data to establish a treatment plan.
• Utilizes exercise, physical agents, and manual techniques to address deficits.
• Maintains thorough documentation with appropriate coding and billing.
• Communicates with other disciplines and referring providers to enhance outcomes.
• Performs thorough electronic documentation.
• Mentors new partners and participates in PI and CQI processes.
• Plans and conducts lectures and training programs for staff and community groups.

Education

Graduate of an accredited school of Physical Therapy
