Job summary

Fallon Health is seeking a CNA/Home Health Aide in Dartmouth to provide personal care, light housekeeping, and ADL assistance in participant homes and at the PACE center.

Under RN supervision, you will help develop care plans, support with therapies and daily activities, and maintain clean, safe environments in line with CDC guidelines. Summit Eldercare competencies are required annually, with potential for full-time benefits and paid time off.

Qualifications

• Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program is highly desirable, but equivalent work experience may be considered.
• Certification as a Home Health Aide or Nursing Assistant is desired; reliable transportation required.
• One year of experience with frail/older adults, preferably in a PACE program; Summit Eldercare competencies required annually.

Responsibilities

• Assist with Activities of Daily Living (ADL) in PACE center and participants' homes under RN supervision.
• Contribute to care plan development with Interdisciplinary Team.
• Maintain infection control and medication/documentation protocols per policy.
• Support meals, mobility, transfers, and activities as prescribed by therapists.
• Document accurately in EMR and other systems per SE procedures.
