Qualifications

• Current state LPN license and CPR certification.
• Graduate of an approved LPN program.
• 1–2 years nursing experience preferred.
• Experience with elderly residents highly desirable.

Responsibilities

• Accurately assesses, communicates, and documents residents’ status.
• Observes and reports changes in resident health to appropriate leadership and family as required.
• Directs and supervises daily work of Resident Care Assistants.
• Mentors Lead Resident Care Assistants in job aspects.
• Coordinates care with community providers via case management.
• Balances service with resident independence.
• Prevents elder abuse, neglect, and exploitation and reports as required.
• Responds to emergencies and activates backup as needed.
