Job summary

Fox Hollow Post Acute is seeking an LPN/LVN supervisor to oversee nursing staff and ensure high-quality resident care in a long-term care setting. Responsibilities include directing daily nursing activities, ensuring policy compliance, and participating in care planning and discharge planning.

The role requires current licensure, CPR, and the ability to lead a team in a fast-paced environment while coordinating with physicians and other departments to optimize resident outcomes.

Qualifications

• Must possess, as a minimum, a Nursing Degree or be a graduate of an approved LPN/LVN program.
• Current, active license to practice as an LPN/LVN in the state.
• CPR certification and knowledge of nursing practices and procedures.

Responsibilities

• Direct day-to-day nursing care by licensed staff and nursing assistants.
• Ensure compliance with facility policies, procedures and regulations.
• Participate in development and execution of the resident care plans and discharge planning.
• Perform administrative duties, charting, and reporting to the Nurse Supervisor.
