Livonia Health Care Center in Livonia, Michigan is seeking a dedicated Licensed Practical Nurse (LPN) to join our team on a full-time day shift (7am-7pm). You will provide direct patient care, conduct assessments, and communicate with residents, families, and the healthcare team.

Responsibilities include admissions, MAR/TAR/POS completion, discharge planning, wound care, and leading CNAs and CMTs while maintaining facility protocols. Licensure as an LPN is required.

Responsabilités

• Conduct assessments, document findings, and report changes.
• Complete admissions duties including MAR, TAR, POS and medication orders.
• Manage discharges and coordinate transportation.
• Provide wound care according to protocol.
• Lead CNAs and CMTs, ensuring compliance with facility policies.
• Respond to emergencies.
