Job summary

Mission Point Health Care in Michigan seeks a licensed practical nurse (LPN) to join our long-term care team for 12 hour day shifts. You will assess residents, implement care plans, and document observations while following policies and procedures.

Responsibilities include medication administration, rounds with physicians, patient education, and coordination with CNAs to support resident comfort and safety. A valid Michigan LPN license and CPR are required; prior LTC experience is preferred.

Qualifications

• Knowledge of current federal and state laws and regulations that apply to the practice of nursing in a long-term care setting.
• Has strong organizational and time management skills and the ability to prioritize responsibilities.
• Ability to apply mathematical concepts such as fractions, percentages, ratios, and proportions to practical situations.
• Basic computer knowledge is required.

Responsibilities

• Assess resident conditions to implement intervention and develop care plans; record observations and follow care plans.
• Make regular rounds of residents with a physician; observe symptoms and reactions to treatments and medications.
• Provide preventative, supportive, maintenance, and rehabilitative care directed to residents’ physiologic and psychosocial needs.
• Administer medications to residents as prescribed and notify personnel of contraindications.
• Advise residents regarding changes in medications and treatment plans; coordinate emergency care when needed.
• Admit, transfer, and discharge residents; ensure supplies and equipment are available.
• Monitor and assist CNAs with personal resident care duties such as bathing and feeding.
