Livonia Health Care Center in Livonia, Michigan is seeking a dedicated Licensed Practical Nurse (LPN) to join our team on a full-time day shift (7am-7pm). You will provide direct patient care, conduct assessments, and communicate with residents, families, and the healthcare team.

Responsibilities include admissions, MAR/TAR/POS completion, discharge planning, wound care, and leading CNAs and CMTs while maintaining facility protocols. Licensure as an LPN is required.

About the Role:

We are seeking a dedicated and compassionate LPN to join our team on a full-time basis, working day shift (7am-7pm). As an LPN, you will be responsible for providing direct patient care, conducting assessments, and communicating effectively with patients, families, and healthcare teams.

Responsibilities and Duties:

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings
• Perform admission duties, including completing assessments, MAR, TAR, POS, reviewing chart set-up, and ordering medications
• Manage discharges, including completing documentation, notifying physicians and guardians, and arranging transportation
• Provide wound care, following protocols, irrigating, packing, and removing dressings
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies
• Respond effectively to emergency situations

• Licensure/Certification: LPN
