MDH Network, Inc. is seeking a Community Support Professional to work full-time in San Diego, CA.

You will provide person-centered support to adults in their homes and community settings, helping with daily activities and routines in a compassionate, respectful manner. The role requires a High School diploma or GED, a valid CA driver’s license, and English proficiency; bilingual in English/Spanish is a plus.

Qualifications

• High School diploma or GED; must be at least 18 years old.
• Experience serving individuals with intellectual or developmental disabilities or behavioral challenges.
• Valid California driver’s license and auto insurance; company vehicle if available and must be 21+.
• Ability to read, write and speak English; bilingual in English/Spanish preferred.

Responsibilities

• Provide person-centered support in participants' homes and community settings.
• Assist participants with daily routines, activities, and transportation as needed.
• Document participant progress and coordinate with the care team.
• Promote independence and meaningful engagement with safe, respectful care.
