MDH Network is seeking a Community Support Professional in San Diego, CA for a full-time, day-shift role. You will support individuals with intellectual or developmental disabilities in their homes and community, following person-centered plans and promoting daily living skills.

The position requires a High School diploma or GED, a valid California driver's license, and English proficiency. Bilingual in English/Spanish preferred.

Qualifications

• Minimum High School diploma or GED.
• Current driver’s license and California automobile insurance.
• Ability to read, write and speak English; bilingual in English/Spanish preferred.

Responsibilities

• Assist individuals in home and community settings with daily routines and activities.
• Train individuals on task-level actions and supervise job-related tasks.
• Document daily notes and provide input to personalize training needs and schedule.
