Responsibilities

• Collaborate with care team of doctors, nurses, and social workers.
• Coordinate care for patients and families across medical and social needs.
• Complete face-to-face visits and evaluate progress of care plans.
• Document care plan activities and communicate next steps to the care manager.
• Monitor ongoing needs to remove barriers and improve access to care.
• Schedule patient appointments and manage calendar coordination.
• Provide updates to families on patient condition and progress; connect to resources.
• Consult with care team on complex issues and support population outreach.
