Job summary

Cincinnati Children’s is seeking a Community Health Worker to join the Social Service department on the Burnet Campus. This full-time role involves day shift with 40 hours per week, coordinating care for patients and families, and working with a multidisciplinary team to address medical and social needs.

Responsibilities include conducting face-to-face visits, documenting care plans, scheduling appointments, and supporting outreach to at-risk populations.

Qualifications

• Candidates must have a high school diploma or equivalent
• 2-5 years of work experience in a related job discipline
• Have a reliable vehicle/valid driver's license

Responsibilities

• Work collaboratively with care team of doctors, nurses, and social workers.
• Coordinate care for patients and families, including medical and social needs.
• Complete face to face visits, evaluate care plan progress; communicate results/coordinate next steps with the care manager.
• Monitor ongoing needs of the patients/families to remove barriers, ensure efficiency and help improve access to care and resources.
• Schedule appointments for members/families.
• Complete required documentation linked to the care plan.
• Provide updates to families regarding patient condition and progress; refer families to appropriate resources for information to answer questions and address needs.
• Consult with care team members to address complex issues.
• Supports population outreach to identify population(s) at risk and assist with closing population gaps.

Education
